Form 1

This form shows a date picker which you can type in or click and it opens a calendar. Dates are limited to start from 2 weeks time up to 3 years (all can be changed). Message box pulls through the date and the page title (so 'Form test' here would be the itinerary name). The Marketing box at the bottom would, if checked, add the user to Mailchimp (this needs to be hooked up to work so won't do it on this demo)
